Assisted Living

Assisted living is for people who need help with daily care, but not as much help as a nursing home provides. Assisted living residents usually live in their own apartments or rooms and share common areas. Assisted living facilities are referred to by different names such as a board and care or RCFE but the terminology is generally determined by the number of residents.

Most people pay the full costs of assisted living themselves. This option tends to be more expensive than living independently but less expensive than a nursing home. Medicare does not pay for assisted living. Medicaid may provide coverage for some aspects of assisted living, depending on the state and whether the person is eligible.

Health Quality Management Group’s current portfolio delivers the full spectrum of assisted living services which includes but not limited to:

  • Housing
  • Meal Services
  • Assistance with Personal Care
  • Medication Assistance
  • Housekeeping
  • Laundry
  • 24-hour On-site Supervision
  • Social and Recreational Activities

BEHAVIORAL HEALTH

Adult Residential Care Facilities (ARF) are specific facilities for adults aged 18-60 with serious mental illness. This level of care is considered non-medical and are not required to have nurses, certified nursing assistants or doctors on staff, but the persons are unable to live by themselves.

Congregate Care

Congregate Living Health Facilities (CLHF) are facilities with 6-15 beds specializing in sub-acute care for a specific population such as patients who are severely physically disabled, patients with 6 months or less to live, or patients with a catastrophic injury due to trauma.

Health Quality Management Group’s current portfolio offers the same services found in a skilled nursing facility such as ventilator, trach, and wound care as well as rehabilitation, meal, and recreational services.

Independent Living

In contrast to assisted living, independent living is for individuals who can still live independently but enjoy having access to assistance when needed. Often called retirement homes, independent living offers residents a safe living environment, but with minimal assistance.

Health Quality Management Group’s current portfolio provides the following in our independent living facilities:

  • Hospitality services such as housekeeping, meals, and laundry are available
  • 24-hour on-site personnel
  • Private apartment
  • Organized staff led activities and outings
  • Transportation to appointments available

Memory Care

Memory Care facilities provide the same services as an assisted living facility such as housing, meals, assistance with medications and personal care, housekeeping, laundry, and activities. However, memory care with increased safety measures, specially trained staff, cognitive therapies, and more, also offers a specialized environment for people with dementia.

Health Quality Management Group’s current portfolio provides the following in our memory care facilities in addition to the basic services:

  • Safety – Locked entrances and exits with keypad entries
  • Staff Training – specialized training to provide person-centered care for people with Alzheimer’s and other dementia related illnesses
  • Amenities such as a spa or relaxation room, library, outdoor spaces, sensory stimulation rooms/activities and pet-friendly accommodations

SKILLED NURSING

Skilled nursing care is most often associated with a skilled nursing facility (also known as a SNF and sometimes referred to as a nursing home, convalescent hospital, or rehabilitation center). This level of care is the highest a person can receive outside of a hospital. A SNF is required to provide 24-hour skilled nursing care, as well as related and/or rehabilitative services. The typical resident is a person who is chronically ill or recuperating from an illness or surgery and needs regular nursing care and other health related services.

Health Quality Management Group’s current portfolio delivers the full spectrum of skilled nursing care which includes but not limited to:

  • Hemodialysis and Peritoneal Dialysis
  • Blood Transfusion
  • Ventilator Care (management and weaning)

  • Tracheostomy Care (management and decannulation)
